Candidate Engagement Agreement

This agreement outlines expectations for professional communication between Eye Spy
Talent and the undersigned optometrist (“Candidate”) while exploring job opportunities.

1. Communication & Responsiveness

The Candidate agrees to communicate in good faith, provide honest updates regarding
interest and availability, and respond to reasonable outreach within 3–5 business days (or
request additional time).

2. Communication Closure

If the Candidate is no longer interested in a role or in working with Eye Spy Talent, they
agree to communicate this decision so the Company can close the loop and proceed
accordingly.

3. Accuracy of Information

The Candidate confirms that all information shared — including licensure, certifications,
clinical experience, availability, and work authorization — is accurate to the best of their
knowledge and agrees to disclose any material changes promptly.

4. No Financial Obligation

This agreement does not require payment from the Candidate and does not obligate
acceptance of any position.

5. Term

This agreement remains in effect while the Candidate is actively working with Eye Spy
Talent and may be ended at any time with written notice.
